Continuous Care is part of an Accountable Care Organization (ACO). This means we work closely with doctors, hospitals, and other health care providers to make sure you receive coordinated, high-quality care.
Our shared goal is simple: help you get well and stay well by making your care easier, safer, and more connected.
How coordinated care supports you
When your health care team works together, you benefit from care that’s centered around you.
With an ACO, you can expect:
- Care focused on your individual needs
- Shared access to your medical information, such as test results, treatments, and prescriptions
- Better coordination to help prevent medical errors and drug interactions
- Fewer repeated tests and appointments, saving you time and frustration
- Improved communication that helps protect against Medicare fraud and waste
Expanded Medicare benefits may be available
As part of an ACO, you may qualify for additional Medicare benefits.
For example, if you need rehabilitation services or skilled nursing facility care, you may be able to receive this care without a hospital stay.
Eligibility varies, so be sure to ask your health care provider if you qualify for these expanded benefits.

We encourage you to take advantage of tools designed to help you stay informed and connected.
- Ask about signing up for our secure online patient portal
- Access your personal health information 24/7, including lab results and messages from your care team
Choosing a health care provider that participates in an ACO helps ensure you get the right care, at the right time.
You can also visit Medicare.gov to log into (or create) your secure Medicare account and choose a primary care doctor.
Your privacy matters
Medicare protects the privacy of your health information.
If you prefer that Medicare not share your information with your health care providers for care coordination, you may opt out by calling:

Please note: Medicare may still share general information to measure provider quality.
